This page is for Survivors who wish to fill in our service evaluation. You should already be receiving support from us and you may have been asked by your Recovery Practitioner or Peer Support Practitioner to complete this form. Your responses let us know what we are doing well and what we could improve, for you, and all Survivors who access our services. Your feedback is valuable because it enables us to demonstrate to funders how our work benefits the Borders community. Your answers may be used in funding applications and reports but will always be anonymised.
